Beware of some of the state issued vaccine exemption forms. Those are modeled after the Refusal To Vaccinate Form and contain very incriminating language. A true exemption is YOUR statement, as opposed to the STATE's assumed FALSE statement. Beware!
A good example is the older incriminating pre-2018 MI State form...
“By signing this waiver, you acknowledge that you are placing your child and others at risk of serious illness should he or she contract a disease that could have been prevented through proper vaccination.”
They have changed the wording on the new 2018 state form, where it is no longer incriminating. STILL, people should be allowed to provide their own statements or forms.
"By signing this waiver, I acknowledge I have attended the waiver session and have been informed about vaccines and vaccine preventable diseases. I also understand that my child may be excluded from the school or childcare center if the local health department determines that it is necessary to control the occurrence of a vaccine preventable disease."
The other inconvenience is HAVING to go to INDOCTRINATION sessions before you can get your exemption.

Another way to to counter the incriminating language in state forms, is that the PDFs can be converted to Word first, and then do slight editing to the statements and print from Word or go back and re-convert to PDF.
